Physical packets must have all signature lines completed by both athlete and parent by May 16. Parents may sign and notarize forms before the event. Athletes may attend alone if all parent signatures on the FHSAA Physical are completed. Only the physical exam can be done without a parent. Other forms requiring a notary need the parent present. If a parent is present, all forms can be completed on-site. Athletes cannot complete the physical without either pre-completed signatures or a parent being present.
